Cosmetic dentistry is a plan built after an examination, in which one or more treatments are chosen according to the condition of each tooth. At Dr. Binyahya Dental Clinic in Hajer District, Dhahran, it covers cleaning and laser whitening, tooth-coloured cosmetic fillings, crowns, bridges and veneers, laser gum trimming and reshaping, and removable dentures. A cosmetic case usually begins with a single observation: a shade that has changed, a chipped edge, a gap between two teeth, or a gum that shows more than the teeth do. Each of these has a different cause and a different answer. A change of shade may be surface staining that a cleaning removes, or it may lie deep within the tooth, where only whitening or a covering on the front surface will alter it. The examination therefore comes first, not the choice of treatment.

Where more than one tooth is involved, the teeth are not treated one by one: the treatments are put in order within a single plan. Treatment comes before cosmetics in that order. Decay is treated, gum inflammation is brought under control, and any root canal treatment or repair the tooth needs is completed first, because anything built on an unsound tooth or gum gives a short-lived result. The cosmetic steps are then sequenced in turn, with whitening carried out before the shade of fillings and restorations is chosen, since their colour does not lighten once they are fixed in place.

All of these treatments are carried out at the clinic: cleaning and laser whitening, tooth-coloured cosmetic fillings, crowns, bridges and veneers, laser gum trimming and reshaping, and removable dentures for missing teeth. Orthodontics is not among the clinic’s services, and neither are facial aesthetic procedures. If the examination shows that a case needs orthodontic treatment to correct the position of the teeth or the bite, the dentist says so rather than covering the problem with restorations.

The dentist examines the teeth, the gums and the bite, and a digital panoramic X-ray is taken in the clinic where needed, so the condition of each tooth and how much of its structure remains are known before any cosmetic step is settled.

Treating decay and gum inflammation, and any root canal treatment the tooth needs, come before the cosmetic steps, because a restoration placed on an unsound tooth or gum may have to be redone before long.

Whitening before the shade of a restoration is chosen

A cosmetic filling or a restoration does not change colour once it is fixed in place. Where whitening suits the case it is therefore done first, and the shade of the restoration is then matched to the new one, so natural and restored teeth sit close in colour.

What the cosmetic plan does not include

The clinic does not provide orthodontic treatment or facial aesthetic procedures. Where the problem lies in the position of the teeth or in the bite, the dentist explains that orthodontics is the treatment for it, and restorations are not used in its place.

Aftercare instructions

  • Brush twice a day and use floss or an interdental brush, because decay at the edge of a filling or restoration, and inflammation of the gum around it, are among the commonest reasons a cosmetic result comes to an end.
  • Cut down on coffee, tea, strongly coloured drinks and smoking, because staining returns to the natural teeth over time, and whitening does not lighten fillings or restorations.
  • Avoid biting hard objects, and do not open packaging or cut thread with your teeth, because the edges of restored front teeth are more liable to chip than others.
  • Tell the dentist if you notice yourself grinding your teeth at night or clenching your jaws, because it affects the choice of treatment and shortens the life of front restorations.
  • Contact the clinic if sensitivity persists, if an edge feels rough, or if the bite has changed, because such things are easier to adjust early than to deal with later.
  • Keep up regular check-ups and cleanings, because that is when the edges of the restorations and the gum around them are examined, before any problem advances.

What is the difference between a crown and a veneer, and when is each chosen?

A crown covers the tooth on every side; a veneer is a shell over the front surface alone. What decides between them is the condition of the tooth rather than the appearance sought. A tooth that has weakened, had its nerve treated or lost a large part of itself needs full coverage, whereas a strong tooth whose problem is its colour or the shape of its front surface may need only a veneer. It is settled tooth by tooth, so both may appear in one smile.

When is whitening or a cosmetic filling enough?

Whitening is usually enough when the teeth are structurally sound and the only problem is their colour. A cosmetic filling is enough when the loss is limited: a chipped edge, slight wear, or a small gap between two teeth. Where a single tooth has several problems at once, where its walls have weakened, or where its colour does not respond to whitening, the dentist may move to a veneer or a crown. The principle is to choose the least that serves the purpose and preserves the structure of the tooth.

Is the case planned in one visit?

The case can be assessed and the outline of the plan set out at the first visit, after an examination and an X-ray where needed. Carrying it out usually takes more than one appointment, and some of its details may change during treatment, for instance if decay appears under an old filling or a tooth turns out to need root canal treatment that was not expected. The dentist explains any change and its reason before going on.

How long does a cosmetic treatment plan take?

There is no single answer, because it follows the number of teeth involved and the type of treatment. A whitening session or a cosmetic filling may be completed in one visit, while restorations need one visit for preparation and impressions and another for the try-in and fitting. If decay, gum treatment or root canal treatment is required beforehand, those appointments are added. The dentist estimates the expected number of visits after the examination.

Is a cosmetic result permanent?

No. Cosmetic treatments usually last many years, and no fixed period can be given for them. The natural teeth take up staining again over time, depending on diet and smoking; fillings and restorations need monitoring and may be replaced in due course; and the condition of the gums changes as well. Daily oral care and regular check-ups are what extend the life of the result, and a patient may need a whitening session or an edge adjusted after a while.

Does the plan include orthodontic treatment?

No: orthodontics is not among the clinic’s services. Orthodontic treatment is what corrects the position of the teeth, crowding and the bite, and restorations do not do that work, since they change the shape and colour of a tooth rather than where it sits in the jaw. If the examination shows a case needs orthodontics, the dentist says so, because covering crowded teeth with restorations can mean removing more sound structure.

What does the term Hollywood smile mean?

It is a common advertising phrase rather than the name of a particular procedure. It is generally used for a group of cosmetic treatments carried out together on the front teeth, such as whitening, cosmetic fillings, veneers, crowns or gum work. The clinic does not treat it as one fixed service; which treatments are used, and how many, is decided for each patient after an examination.

Can an uneven gum line be treated along with the teeth?

It may form part of the plan, since the gums are assessed together with the teeth: the shape of a smile comes from both. Where excess gum tissue is the reason the gum shows, it can be trimmed and reshaped with the laser before any restorations or fillings. Other causes are not treated by trimming the gum, among them a high upper lip, the shape of the jaw and the position of the teeth, so the dentist establishes the cause at the examination first.
